Person Centered Planning and Implementation for Providers (Part A & B)

The purpose of Person Centered Planning and Implementation for Providers is to refocus our service planning and implementation processes using person centered approaches to ensure that the people we serve and their families are directing the choice and control over the services and supports each person receives.  It will enhance the provider’s ability to ask people about their life expectations, listen and understand the person’s desired outcomes and then construct and put supports and services in practices that honor those personal choices.  The training will consist of two sessions (Part A & B).  Both sessions need to be taken to complete the requirement.
