Enter the Category that best describes your status.
Examples:
- New Applicant - you are not currently enrolled or have a signed agreement with APD
- Enrolled Independent Provider - A provider has a current signed agreement and enrollment listing with APD
- Enrolled Agency Owner - An Agency owner that has a current signed agreement and enrollment listing with APD
- Employee of an Agency - You are currently an employee of an Agency (includes APD)
- Other/person receiving services, family members, guardian, advocates - Includes persons receiving services, family members, guardian, and advocates
