The Medicaid Waiver Waitlist

The Agency for Persons with Disabilities maintains a statewide waiting list of individuals requesting services under its Developmental Disabilities Home and Community-Based Services Waiver.  The number of individuals added to the waiver each year depends upon several factors. The factors that affect enrollment include:

• Funding availability
• Legislative requirements
• People in crisis have first priority
• Children in the Child Welfare System are given second priority

As funding becomes available, the agency will enroll people from the waitlist.

Intake and Eligibility

To receive services, an individual must have a diagnosis of cerebral palsy, intellectual disabilities, autism, spina bifida, or Prader-Willi syndrome.  The Area 4 Intake Workers are responsible for collecting the documentation needed verify the diagnosis.  With permission of the individual or guardian, this documentation may be obtained from school records, psychological reports, and medical evaluations.  If this information is not available form other resources, the Agency for Persons with Disabilities can provide these evaluations at no cost to the individual or family.

Area 4 Intake Workers

General Revenue Support Coordinators for Area 4:

Everyone on the waiting list has a support coordinator. The Area 4 General Revenue Support Coordinators are responsible for maintaining information related to people on the APD waitlist. This includes their address and telephone numbers as well as information related to the individual’s need for services.  The support coordinator may also assist by identifying community resources that may helpful to the individual while they are waiting for services from the Medicaid Waiver program.  Below is a list of the support coordinators in Area 4.
