Applying for Services
People who have autism, cerebral palsy, Down syndrome, intellectual disabilities, Prader-Willi syndrome or certain forms of spina bifida may apply for services from the Agency for Persons with Disabilities, which includes waiver participation or placement in a Developmental Disabilities Center (DDC). Please contact the APD office that serves your area for more information and to submit an application for services.

Consumer Directed Care Plus (CDC +)
CDC+ is a long-term care program alternative to the Medicaid Home and Community-Based Services (HCBS) Medicaid Waiver. The program provides the opportunity for individuals to improve the quality of their lives by being empowered to make choices about the supports and services that will meet their long-term care needs and to help them reach their goals.
Emergency & Disaster Information
APD suggests developing an evacuation plan, a communication plan, and obtaining general disaster supplies. However, a person with a disability may have additional planning to do. Don't put off until tomorrow what you can plan for today!
