Procedures for Existing Providers to Provide Services on all Tier Waivers

In order for an existing provider to provide services to individuals assigned to the four (4) Tier Waivers, the following provider enrollment procedures will need to be put in place. Applicants who are not currently providers must contact the Area APD office for enrollment information.

  1. In order to provide services to someone in Tier Waivers 1, 2 or 3: The provider must have a DD waiver provider number. If the provider has an FSL provider number and wishes to also provide services to individuals on Tier Waivers 1, 2 and 3, the provider will have to apply for a new locator code per the procedures, and using the forms in the email below. This information must be submitted by the Area enrollment specialist to Margie Collins at the APD central office for processing.


  2. In order to provide services to someone in the Tier 4 (formerly the FSL) waiver: The provider must have an FSL waiver provider number. If the provider has a DD provider number and wishes to also provide services to individuals on the Tier 4 Waiver, the provider will have to apply for a new locator code per the procedures, and using the forms in the email below. This information must be submitted by the Area enrollment specialist to Margie Collins at the APD central office for processing.


  3. If the provider does not want to expand their service to all waivers, or the provider currently has provider numbers for both the DD and the FSL waivers, they are good to go. Providers with provider numbers for both the DD and the FSL waivers may provide services on Tier Waivers 1, 2, 3 and 4. The appropriate provider number and procedure codes must be used when billing for people enrolled on the various waivers. The DD waiver provider number and procedure codes must be used for Tier Waivers 1, 2 and 3; The FSL provider number and procedures codes must be used for Tier 4.

    ACS will process provider applications until June 6, 2008. EDS will begin processing provider applications on July 1, 2008. There will be a slight gap in the application approval process due to the AHCA change in providers. Please encourage any providers wishing to seek a new locator code to initiate the application process as soon as possible.

    Forms to apply for a new locator code may be obtained from the Area office Provider Enrollment Specialist or on the APD web site if the provider is currently an active HCBS provider (either DD waiver or the FSL waiver.) All applicants must have been background screened within the past 12 months of application to be eligible to apply. All forms must have original signatures and the letter from the bank must be an original. A copy of the qualification memo needs to also be attached to the packet.
