Interested in applying to license a new foster home, group home, or Adult Day Training Program?
Individuals or entities interested in opening a licensed foster home, group home, or Adult Day Training program must have a facility secured and available at the time of application. If the potential applicant has a facility secured and available and is interested in seeking facility licensure, the applicant may click on the link below to request an application. This link is not for general inquiries.
All APD providers and direct service providers are required to pass a Level II Background Screening in accordance with Section 435.12 and 393.0655 Florida Statutes.
Step 1: Click on the Application Request Link below and complete all fields on the form. Please attach a copy of a valid photo identification with your request.Step 2: Once you complete and submit this request form, you will be contacted by APD regional staff to confirm your request and provide information about how to submit your license application in iConnect.
Step 3: Submit the completed application and supporting documentation, including evidence of the applicant’s financial ability to operate the facility for up to 60 days without dependence upon payment from the state or other third-party fees from facility residents or participants. Such evidence shall include bank account statements, pay stubs, documentation of a line of credit, or any other documents which would demonstrate the expected ability of the applicant to continue operations for that time period and under those conditions.
Step 4: Provide any additional information or corrections requested by the Agency within 30 days of receipt of the request for additional information.
Step 5: Once the application has been reviewed and is complete, as determined by the regional office, the regional licensure specialist will conduct a facility inspection. Note, the agency must complete facility inspection to ensure that the facility or program meets the physical and safety standards, prior to the issuance of a license.
Step 6: A survey of the proposed facility will also be conducted to ensure compliance with federal Home and Community Based settings requirements. This is required for residential and Adult Day Training facility operators to receive Medicaid Waiver funding for services provided.
