Questionnaire for Situational Information

The Questionnaire for Situational Information (QSI), Version 4.0, is the Agency for Persons with Disabilities’ (APD) approved functional needs assessment.  It collects key information about an individual’s life situation—covering functional, behavioral, and physical needs—to assist in planning services and supports. The QSI also plays a role in determining iBudget allocations for individuals enrolled in the Waiver.

The QSI is required at the time of eligibility determination for Waiver services, at least once every three years, and whenever an individual experiences significant life changes that may impact their support needs.
