Support Coordination
Individuals who receive a home and community-based services (HCBS) waiver choose a Waiver Support Coordinator (WSC) who assists the individual in accessing supports and services. WSCs facilitate regular person-centered planning with the individual to identify needs and goals.
Section 393.0663, F.S., requires that all WSCs work for a Qualified Organization (QO).
Waiver Support Coordination
Waiver Support Coordination includes ongoing case management to ensure that clients access services needed to maintain their health, safety, and well-being. WSCs are responsible for assisting individuals in accessing services from all available resources and monitoring the effectiveness of home and community-based services. They advocate for individuals and work to identify, develop, coordinate, and access supports on the client’s behalf. For more information, visit the Waiver Support Coordination webpage.
Qualified Organization
Qualified Organization means an organization that employs four or more WSCs. QO’s render support coordination services to individuals receiving HCBS services. They must meet all specifications in s. 393.0663, F.S.; Rule Chapter 65G-14, F.A.C.; and the iBudget handbook. For more information, please visit the Qualified Organizations webpage.

Additionally, the Agency has created templates for required policies and procedures, mentoring program, disciplinary process, and code of ethics for Qualified Organizations to use if they choose. Prospective qualified organizations may:
- Use the templates developed by APD;
- Qualified Organization Mentoring Program Template
- Qualified Organization Disciplinary Process Template
- Qualified Organization Code of Ethics Template
- Qualified Organization Policies and Procedures Template
- Create their own policies and procedures, mentoring program, disciplinary process, and code of ethics to comply with the aforementioned statute and rule chapter; or
- Use the editable Word versions of the templates to modify to best suit the Qualified Organization.
These documents are required to be submitted pursuant to Chapter 65G-14, F.A.C., and the iBudget Handbook with the qualified organization application.
Application Submission
The Qualified Organization Application must be completed in its entirety and submitted electronically with all required documents to the provider’s APD Home Region.
