To apply to be an iBudget Waiver service provider, please take the following steps:

  • Download the appropriate APD application here.
  • Fill out application thoroughly and legibly.
  • Gather all supporting documentation (utilizing the checklist contained inside the application).
  • Submit the application and supporting documentation to the Provider Enrollment email address below for the region where you are applying to render services:

In order to expand, Providers must meet the minimum qualifications per the iBudget Handbook.
