Medication Administration
Agency for Persons with Disabilities (APD) clients who reside in APD-licensed facilities , supported living settings, and/or attend adult day training programs may receive required medication assistance from trained, unlicensed direct service providers (Medication Assistance Providers) per Chapter 393, Florida Statutes and Chapter 65G-7, Florida Administrative Code, Medication Administration. Medication Administration Trainers
The APD 65G-7 Medication Administration Trainer Training Classroom Course is only available for nurses licensed per Chapter 464, Florida Statutes, which includes Licensed Practical Nurses (LPNs), Registered Nurses (RNs) and Advanced Practice Registered Nurses (APRNs).
Prospective trainers should reach out to their local APD Regional Office for additional information regarding requirements. The course should be taken within the region where trainers will be teaching the Basic Medication Administration Course. The contact information for each regional office is listed below:

- Validation Trainer Application APD Form 65G-7.003 - Validation Trainer Application Form
Note: This is for trainers who desire to perform validations only. - Copy of nursing license
Important notice: This course is for trainers who will be teaching the Basic Medication Administration Course to APD unlicensed direct care staff only. Unlicensed direct care staff who wish to become Medication Assistance Providers (MAPs) should contact an APD-approved Medication Administration Trainer for information about registering for the Basic Medication Administration Course
Medication Administration Forms
For APD consumers residing in APD-licensed group homes, supported living setting and/or attending adult day training programs requiring medication assistance, the following forms must be used as applicable per Rule 65G-7 Medication Administration, Florida Administrative Code.
Each APD consumer must have a signed Authorization for Medication Administration Form (APD Form 65G-7.002A) on file. APD consumers requiring supervision of self-administration of medication and assistance with medication assistance must also have a completed Informed Consent for Medication Administration Form (APD Form 65G-7.002B) on file. All medications administered must be documented on the Medication Administration Record (APD Form 65G-7.008). Additionally, the administration of controlled medications must be documented on the Controlled Medication Count Form (APD Form 65G-7.007B)
Please note: Pharmacy-generated and electronic versions of the Medication Administration Record are acceptable if they meet requirements referenced in Rule 65G-7.005.
