Visitation Information

  • Upon admission to DDDP/Pathways, Social Services staff will meet with the resident and identify who they would like to have as contact persons and who they would like to have visit them during their stay at DDDP/Pathways.

  • Once the resident signs a release of information allowing contact with these individuals, contact is made and a visitation packet is mailed to them. Included in the packet is information about DDDP/Pathways, and our policy on visitation, and an application for visitation. The information instructs the individual to complete and return the application for visitation for approval by our Security Department. Once the individual is approved by security, they can contact Social Services staff to arrange visitation.

  • Visitation hours at both facilities are Saturday and Sunday from 9 a.m. to 11 a.m., and then again from 1 p.m. to 3 p.m.
